The preliminary account of the September 21 crash

A motorcyclist died in a three-vehicle collision on westbound State Route 4 in Concord on Monday, September 21, according to a California Highway Patrol account republished by ContraCosta.news on September 23. Officers responded around 3:06 p.m. east of Port Chicago Highway.

CHP’s preliminary account described a collision between a Volkswagen Jetta and the motorcycle. The rider was ejected and then struck by a Toyota Tundra, the release said. The rider was pronounced dead at the scene. No other injuries were reported in that account.

No other injuries were reported. Officers did not suspect alcohol or drugs at that stage of the investigation.

What the latest coverage adds, and what it does not

The Contra Costa Herald published a further account on September 24. Its description of the collision draws on the same CHP release, so the two articles should not be counted as independent confirmation of the sequence. This is coverage of Monday’s crash, not a new collision on September 25.

A description of which vehicles made contact is not, by itself, a final explanation of why the collision happened. The material reviewed here does not establish a final fault determination. The September 23 account asked witnesses to contact CHP’s Contra Costa office at 925-387-1900.

For someone considering whether they have useful information, the relevant reference points are the westbound direction, the area east of Port Chicago Highway, and the afternoon of September 21. A witness can describe what they personally observed and let the investigating agency ask follow-up questions. There is no need to decide responsibility before contacting the office.

Finding the right route to the collision report

Because this investigation is being handled by CHP, its crash-report request process is the relevant starting point. CHP provides an online Crash Portal for eligible parties of interest. It also accepts requests in person or by mail using form CHP 190.

Eligibility matters. CHP lists several qualifying categories, including involved drivers and passengers, parents of minors, legal guardians and authorized representatives. A family relationship alone should not be treated as confirmation that a person can obtain a particular record. Ask the agency which category applies and what documentation it requires.

The request instructions ask for the incident date and location, the requester’s information and signature, and the applicable fee. Before submitting, check that the request identifies the September 21 collision on westbound State Route 4 east of Port Chicago Highway rather than a different incident elsewhere on the highway.

Questions to organize before making the request

Useful questions include whether the report is available, which request method is appropriate, and what is needed to establish eligibility. A short checklist can help a family member or representative keep track of the answers. These are practical suggestions for navigating the published process, not a guarantee of access or a prediction about the investigation.

It can also help to keep the published account and the official record in separate folders. The news coverage explains what was publicly reported; a report request seeks the agency’s documentation. If the two later appear to differ, identify the specific detail and ask for clarification rather than assuming the earlier article settles it.

The distinction is particularly useful here because the available news accounts share a source. A second headline repeating the same description does not supply a second witness account. Before drawing conclusions, a family can ask what additional information has actually become available.

Reporting and sources

The incident narrative comes from a CHP release republished by ContraCosta.news and the Contra Costa Herald. These are one reporting origin for corroboration purposes. CHP’s report-request page is linked as a practical resource, not as independent verification of the crash.
